Home Visit Connections

toddler-playingHome Visit Connections is a referral network for community partners who provide home visits to pregnant women, young children and children birth to 21 with a disability. Participants include Families First, the North Central Public Health District Home Visiting Program, Early Head Start, Early Intervention, Columbia River Women’s Clinic and Mid-Columbia Medical Center’s First Impressions Birthing Center.

The mission of Home Visit Connections is to coordinate agencies so you and your family get the most appropriate home visiting services.
