Notifiable Conditions
Oregon clinicians are legally required to report certain conditions to the health department.

To make a report, fill out the Notifiable Condition Report Form (below) and fax it to 541-506-2601.
You can also call 541-506-2600 and ask to speak with a Communicable Disease Investigator. Our office hours are Monday thru Friday from 8:30a.m.-12:00p.m. and 1:00p.m.-5:00p.m.
***If you need to report a serious or highly contagious disease outside of working hours, Communicable Disease staff are on call at 541-993-9684. If no one answers, leave a message and someone will return your call within a few hours.***
