This is for non-commercial permits and evaluations with daily sewage flows less than 1000 gallons per day, please contact Environmental Health at (541) 506 2601 for commercial applications or if you have a sewage flow greater than 1000gpd.

Online prices are inclusive of a 3% online processing fee.

Each of the above fees includes a $100 DEQ surcharge that will be forwarded to the State Department of Environmental Quality.
